The Timing of Diskectomy as a Predictor of Outcomes in Patients With Lumbar Disk Herniation

This prospective cohort study evaluates how the duration of preoperative symptoms influences outcomes after microdiskectomy for lumbar disk herniation. Outcomes measured include radicular pain, low back pain, disability (ODI), and motor and sensory deficits, with follow-up at 6 weeks, 6, 12, and 24 months in 1,120 analyzed patients.

Key finding: surgical intervention within one month of symptom onset yields faster, more pronounced pain relief and neurological recovery, whereas symptoms persisting beyond two years associate with the highest rates of residual motor and sensory deficits at 24 months.

Objective Evaluate how the preoperative duration of low back pain, disability, radicular leg pain, and motor/sensory deficits relates to postoperative outcomes after microdiskectomy for lumbar disk herniation (LDH).

Design/setting Prospective cohort (April 2015–October 2022) of adults undergoing microdiskectomy for symptomatic LDH with imaging-confirmed herniation consistent with symptoms.

Assessments & follow-up Pain measured by NRS, disability by ODI, motor deficit by MRC grading, sensory deficit as present/absent; assessed pre-op and at 6 weeks, 6, 12, and 24 months post-op.

Cohort size 1222 patients enrolled; 69 recurrences and 33 lost to follow-up excluded, leaving 1120 for statistical analyses.

Symptom-duration grouping Patients categorized by symptom duration thresholds ≤1, ≤6, ≤24, and ≥25 months for LDH-related symptoms (pain/disability/neurologic symptomatology).

Key outcome (timing) Surgery performed within 1 month of symptom onset was associated with more favorable outcomes than longer symptom duration, including faster improvement in neurological deficit and more significant radicular pain relief.

Residual deficits at 2 years Residual motor and sensory deficits at 24 months were least common when preoperative neurological symptoms lasted ≤1 month, and more common when symptoms exceeded 1 month.

Worst outcomes with long duration LDH symptoms persisting beyond 2 years were associated with the least favorable postoperative outcomes, including the highest residual motor and sensory deficits at 2 years.

Motor Recovery Depends on Timing of Surgery in Patients With Lumbar Disk Herniation

Although approximately half of the patients undergoing lumbar disk surgery present with motor deficits, timing of surgery for radicular weakness is largely unclear.

OBJECTIVE: To evaluate the impact of surgical timing on motor recovery in patients with lumbar disk herniation (LDH) and to identify an ideal time window for intervention.

METHODS: In a single-center observational trial, 390 patients with LDH-associated motor deficits were prospectively followed for a minimum of 12 months after nonelective mi- croscopic disk surgery. The duration of motor deficit before surgery was documented. Motor function was graded according to the Medical Research Council (MRC) scale. Statistical analysis of motor recovery applied unbiased recursive partitioning conditional inference tree to determine cutoff times for optimal surgical intervention. The slope of recovery calculated as the change of the MRC grade over time served as the primary outcome.

RESULTS: A preoperative motor deficit of MRC ≤2/5 and the duration of paresis were identified as the most important predictors of recovery (P < .001). Surgery within 3 days was associated with a better recovery for both severe and moderate/mild deficits (P = .017 for MRC ≤ 2/5; P < .001 for MRC > 2/5; number needed to treat [NNT] <2). A sensitivity analysis in mild motor deficits indicated a cutoff of 8 days.

CONCLUSION: Timing of surgery is crucial for motor recovery in LDH-associated deficits. Immediate diagnosis, imaging, and referral should be aimed for to allow disk surgery within 3 days in patients with severe and moderate radicular weakness. If functionally disabling, even mild deficits may warrant decompression within a week.

 

Are There Any Risk Factors Associated with the Presence of Cauda Equina Syndrome in Symptomatic Lumbar Disk Herniation?

Risk factors for developing cauda equina syndrome (CES) caused by lumbar disk herniation (LDH) remain controversial and have not been established yet. The aim of the study was to investigate whether there is a relationship among age, sex, body mass index (BMI), or the degree of spinal canal compromise and the presence of CES in patients undergoing lumbar microdiskectomy.

METHODS: Between 2015 and 2019, 506 patients who had an operation for LDH compressing the dural sac were prospectively identified. The “prolapse-to-canal ratio” (PCR) was calculated as a proportion of the cross-sectional area of the disk prolapse compared with the total crosssectional area of the spinal canal.

RESULTS: In total, 35 CES (6.9%) patients were identified. Multivariate logistic regression, adjusted for age, gender, BMI, and PCR, shows that only PCR was associated with the presence of CES (P < 0.001, area under the curve 0.7431). BMI was not associated with an increased risk of CES.

CONCLUSIONS: This study demonstrates a significant correlation between the size of LDH relative to size of the spinal canal and the presence of CES. A finding of LDH causing >60% obstruction of the spinal canal should be considered a red flag, and such patients need to be watched more closely.

Tubular Diskectomy vs Conventional Microdiskectomy for the Treatment of Lumbar Disk-Related Sciatica: Cost Utility Analysis Alongside a Double-Blind Randomized Controlled Trial

Conventional microdiskectomy is the most frequently performed surgery for patients with sciatica caused by lumbar disk herniation. Transmuscular tubular diskectomy has been introduced to increase the rate of recovery, although evidence of its efficacy is lacking.

OBJECTIVE: To determine whether a favorable cost-effectiveness for tubular diskectomy compared with conventional microdiskectomy is attained.

METHODS: Cost utility analysis was performed alongside a double-blind randomized controlled trial conducted among 325 patients with lumbar disk related sciatica lasting .6 to 8 weeks at 7 Dutch hospitals comparing tubular diskectomy with conventional microdiskectomy. Main outcome measures were quality-adjusted life-years at 1 year and societal costs, estimated from patient reported utilities (US and Netherlands EuroQol, Short Form Health Survey-6D, and Visual Analog Scale) and diaries on costs (health care, patient costs, and productivity).

RESULTS: Quality-adjusted life-years during all 4 quarters and according to all utility measures were not statistically different between tubular diskectomy and conventional microdiskectomy (difference for US EuroQol, 20.012; 95% confidence interval, 20.046 to 0.021). From the healthcare perspective, tubular diskectomy resulted in nonsignificantly higher costs (difference US $460; 95% confidence interval, 2243 to 1163). From the societal perspective, a nonsignificant difference of US $1491 (95% confidence interval, 21335 to 4318) in favor of conventional microdiskectomy was found. The nonsignificant differences in costs and quality-adjusted life-years in favor of conventional microdiskectomy result in a low probability that tubular diskectomy is more cost-effective than conventional microdiskectomy.

CONCLUSION: Tubular diskectomy is unlikely to be cost-effective compared with conventional microdiskectomy.

Long-term Outcome After Microendoscopic Diskectomy for Lumbar Disk Herniation: A Prospective Clinical Study With a 5-Year Follow-up

Several authors have reported results obtained with the microendoscopic diskectomy (MED) technique, but the long-term outcome has not been described. This report summarizes our clinical experience with the lumbar MED technique with a long-term follow-up period.

OBJECTIVE: To evaluate the efficacy of the MED for lumbar disk herniation and to report long-term outcome and complications (5-year follow-up).

METHODS: One hundred twenty consecutive patients with lumbar disk herniation were treated with the METRx system.We included all types of lumbar herniated disks: contained, not contained, foraminal, and migrated disk herniations. The results were evaluated with the Visual Analog Scale (VAS) pain score, Oswestry Disability Index score, patient satisfaction questionnaire, and modified Macnab criteria.

RESULTS: The average age of patients was 41 years; 65 were men and 55 were women. The most commonly affected level was L5-S1 (54.2%). The follow-up time after surgery was 5 years in all cases. We obtained good or excellent results in 75% of patients and regular results in 18%. Good subjective satisfaction was observed with surgery in 92% of patients. The mean decrease in the Oswestry Disability Index score was 52.8 ± 21.6; the mean decrease in leg VAS score was 6.1 ± 2.3; and the mean decrease in lumbar VAS score was 1.9 ± 3.3. Adjusted mean differences were statistically significant in all cases (P < .05).

CONCLUSION: MED not only reduces the incision, tissue damage, and postoperative period of incapacity but also offers long-term results comparable to those of conventional techniques.
